  • Follow the online instructions to complete the Student Employment Packet.
    • You must fill out the packet online.
    • Print it, fill out (in ink) any additional items that were not fillable online.
    • Sign each page (in ink).
    • Do not print first and then fill out by hand.

  • Get a ‘Blue Job Assignment Card’
    • Christine will help prepare the ‘Blue Card’ for you.

If you cannot make it during her drop-in hours, you can email Christine with your name, student ID number, your supervisor’s name, and several dates and blocks of time that you might be available for an appointment. If possible, she will schedule an appointment for you to come in with your paperwork. johnsonchristine@fhda.edu

Remember, whenever communicating (email, phone etc.) with Foothill Hiring Staff include:

  1. Your Full Name
  2. Your Campus Wide ID#

Bring Blue Card to Your Supervisor

Deliver your completed Blue Card to your Music Tech Supervisor. We will deliver to Simon Pennington, Dean of the Division of Fine Arts and Communication, and help followup on the status of your application.

Note to Supervisors

  • Date: If you would like the student to start ASAP after Christine receives the card, just put a small "asap" next to start date and I will make the start date the date I receive it (we date stamp them when they come in to our front desk). If you would like a later start date, please put in the specific date.
  • Student Title - "Lab Assistant"
  • Level
    • Since the Level determines the salary, you would need to put in Level I, II, III, or IV. Guidelines on what level a student should be at based on the duties they will have can be found in the Student Employment Policy Manual. A link can be found in the first paragraph of the Student Employment webpage: https://foothill.edu/aid/employment.php. A description by level starts on page 14.
    • The current hourly pay scale for Levels I, II, III, IV are $10, $12, $14, $16 respectively. In January 2017, Level I will increase to $10.50.
  • Index (not "Fund") Code:
    There is an index code that goes with each FOAP. You use the Index Code for the FOAP you are charging the student’s salary to (if you are paying salaries with Perkins, you would use a Perkins FOAP). You can find your index code by doing an iFOAP search (for Perkins, Donna would probably be your best source): http://business.fhda.edu/accounting/ifoap-search.html
  • Org. Code:
    The Org. Code is the Organization code (the “0” in FOAP) that goes with the Index code you are charging the salary to.
  • Dept. Org Code (to reflect location of employment (required):
    This would be the Organization Code from your department’s general FOAP. It’s used on some reports to indicate the location where the student is working. Your department administrative assistant might be a good source for this number, or you could do an iFOAP search using the budgeteer’s last name.
  • EID# - Employee ID (CWID college-wide identification number)
  • When you using Perkins funds the budgeteer (Dean of Fine Arts) and the Perkins budgeteer (VP of Workforce) to sign it.
  • Fingerprinting Required?
    Unless the job falls into one of the categories described under the yes/no spaces, it is up to the supervisor. The only other time we require a student to be fingerprinted is if they have had a prior conviction (#5 on the student employment packet). In those cases, check “yes.”
